If, after reviewing the website for presenting issues I work with and my financial policies, you would like to discuss possibly working together email me at email@example.com. Only after communication between us do you move forward with approved scheduling as a new client.
Once approved as a new client, scheduling is done using the blue"Schedule Now" button on the bottom of every page. Please get prior approval via email before scheduling. First sessions last between 50-70 minutes. Before the time of your first appointment, please review and complete one of the following history forms. The business agreement form will be in your welcome email. Bring both the history form, the business agreement and payment to our first meeting. These steps allows us to use your time to get right to work rather than spending your visit here on paperwork.
If you have an insurance policy I contract with (Humana, Medical Mutual of Ohio and most Tricare) be sure to have Kate Wolf (513-233-0020, ext 2) verify your benefits prior to session or the cost of our first session may fall to you.

The week before our initial meeting, you will receive via email the introductory business agreement needed to work together. You will also need one of the below forms (depending on the age of client) for that first meeting.
- Adult Personal History (Acrobat). This is for all adult clients to complete. In the case of couples therapy, both adults should individually complete forms. This form assists with assessment and allows our first session to be more focused.
- Child and Adolescent Developmental History Form (Acrobat). To be completed by parents/guardians for new clients that are under 18 years of age.
Optional - If you would like me to coordinate care with another provider (for example, your psychiatrist, endocrinologist, etc.), complete this form:
- Consent to Release Information Form (Acrobat). Only if you know you'll want me to coordinate your care with an agency or person.
