$125 per 50-60 minute session, with the initial session costing slightly more ($150).
Services may be covered in full or in part by your health insurance or employee benefit plan. I am contracted to serve the following plans: Humana, Medical Mutual of Ohio, Healthspan and Private Healthcare Systems. I also may be able to serve as an out-of-network provider with other policies (United Health Care and others) but given the changing face of insurance we would need to check case by case.
My billing provider is Secure Office Solutions with Kate Wolf. Kate handles my accounts and can help check your coverage. Kate can be reached at the office phone 513-233-0020, ext 2. Kate will check my network status and your current benefits at the time of scheduling your intake; that helps the client manage the current maze of managed care with less stress. It is REQUIRED that you contact Kate prior to meeting with me, 48 hours prior is ideal as insurance companies love to put her on hold for extended amounts of time. If I do not have verified benefits at the time we meet you will be expected to cover costs in full for that session. We cannot guarantee coverage even if Kate verifies it in advance. But without advance authorization often our time can not be covered.
The one exception to the above is Anthem BS/BS. Unfortunately, for Anthem you would need to be an informed consumer and check what out of network coverage you have, if any, on your own. Their system requires unreasonable efforts to secure out of network coverage for a provider's office. They are sometimes more responsive to their policy holders. Use the questions below when you call to get the information from Anthem. Stress that you need information about out of network coverage. Write down the answers and bring with you to the first session.
Even with Kate's help it is highly recommended that you check your coverage carefully by asking the following questions:
- Is Cymbria Hess, IMFT and in or out of network provider for my policy?
- What is my deductible and has it been met?
- Do I have a copay or percentage to pay at time of service?
- How many sessions per calendar year does my plan cover?
- What is the coverage amount per therapy session?
- How much does my plan cover for an out-of-network provider?
- Is mental health or out of network a separate deductible?
Cash, check and all major credit cards accepted for payment.
I see clients by appointment Monday, Tuesday, Wednesday, Thursday and random Fridays.
