First sessions last between 50-70 minutes. Before the time of your first appointment, please review and complete one of the following history forms. The business agreement form will be in your welcome email. Bring both the history form, the business agreement and payment to our first meeting. These steps allows us to use your time to get right to work rather than spending your visit here on paperwork.
Expect us to listen, help you to formulate a plan and support you along your journey to better relationships, improved mood and overall improved mental wellness. Ask us questions, participate in setting goals and be accountable to try new behaviors, create new patterns and mostly enjoy the process. Cymbria Hess is contracted only with Humana, Medical Mutual of Ohio and most Tricare. Be sure to have Kate, the client care coordinator (513-233-0020, ext 2) verify your benefits prior to session or the cost of our first session may fall to you.
Barbara J Davis is able to see CareSource Clients. CareSource requires 7 days notice to preauthorize sessions.
All other policies are out of network for our group. Use this form/link - Out of Network Information Gathering Form. to determine the coverage directly with your policy. This means that you would pay your therapist directly and they will provide you with a "Superbill" that you can submit to your insurance to have them reimburse you directly. Use the Gathering form to determine what that reimbursement might be. Or you could elect to not utilize your insurance and therefore have complete confidentiality. Insurance companies require us to share treatment plans, diagnosis and other private information often just to turn around and deny coverage. If you are self-pay privacy is the confidentiality of the therapy relationship soley.
Before our initial meeting, you will receive via email the introductory business agreement needed to work together. You will also need one of the below forms (depending on the age of client) for that first meeting.
- Adult Personal History (Acrobat). This is for all adult clients to complete. In the case of couples therapy, both adults should individually complete forms. This form assists with assessment and allows our first session to be more focused.
- Child and Adolescent Developmental History Form (Acrobat). To be completed by parents/guardians for new clients that are under 18 years of age.
Optional - If you would like us to coordinate care with another provider (for example, your psychiatrist, endocrinologist, etc.), complete this form:
- Consent to Release Information Form (Acrobat). Only if you know you'll want me to coordinate your care with an agency or person.
